Membership
How it works...
To be eligible for our member benefits, simply complete our Membership Application, and mail it to us.
One-time Membership cost:
- Individual Membership: $30
- Family Membership: $40
- Transferring from another FCA affiliate group: $15
Members receive:
- Planning forms and wallet cards to help others know your wishes.
- Contact information for our cooperating funeral director.
- Our newsletter and invitations to Chicago Memorial Association events.
- Information about related topics -- such as your legal/consumer rights, Living Wills/Power of Attorney, and organ/body donation -- upon request.
Participation is simple:
- Complete the planning forms. Keep one copy in your home, return one copy to us for our files, and make additional copies for your next of kin.
- At the time of death, your relatives simply call (312) 939-0678. They will be immediately connected with a cooperating funeral director who will provide service at membership prices.
- If death occurs out of town, you may contact the national Funeral Consumers Alliance at 1-800-765-0107.
- If you move, your membership can usually be transferred to another affiliate for a nominal charge.
