home. Procure the original death certificate, signed by my physician
and a cremation permit signed by
my next-of-kin or other authorized person.
Take my body to the crematory in a suitable cardboard container
for cremation and provide a suitable cardboard container for cremains.

Direct Burial:
(Includes Transportation)

Call for my body and
transport it to the mortuary. I wish it to be known to my relatives
and friends that I request no viewing of my body after death. I
also request no embalming of my body.
Procure the original Death Certificateand Burial Permit.
Provide a particleboard container for my body or $200.00 credit
on a more expensive casket if desired.
Arrange a time convenient for the mortuary and cemetery to inter
my remains.
Provide transportation of my remains to any of the local cemeteries.
Notify Social Security and provide Veteran's Administration forms
if needed.
[Cemetery expenses, Burial plot, Graveliner, Opening and Closing
of Grave, Grave marker are not included].

Economical

Upon my death, my representative
is to call the funeral home listed on my FCA Identification card.
The funeral home will then do as follows:
Call for my body and transport it to the mortuary.
Care and preparation for possible viewing.
Provide a cloth covered flat top casket,or $200.00 credit on a more
expensive casket if desired.
Use of mortuary chapel for funeral service with recorded music.
Provide transportation of my remains to any of the local cemeteries.
Procure the original Death Certificate and Burial Permit.
Notify Social Security and provideVeteran's Administration forms
if needed.
